DNA Testing Could Predict Blood Cancer Progression

Recent research indicates that analyzing DNA patterns may help identify whether certain blood cancers are advancing long before conventional tests can detect those changes. This study, published in Cancer Discovery by the American Association for Cancer Research, investigated 30 individuals with myeloproliferative neoplasms (MPNs). MPNs are a collection of blood cancers where the bone marrow produces an excessive number of blood cells.

Research Findings

Researchers tracked DNA changes in patients’ blood and bone marrow. They studied whether these genetic alterations corresponded with blood counts, disease stability, or progression to myelofibrosis or acute leukemia.

Individuals maintaining stable conditions generally displayed stable DNA, without cancer-associated genetic shifts. This might mean a genetically “quiet” MPN is more likely to stay stable, based on findings from the Wellcome Trust Sanger Institute in the UK.

Patients with worsening conditions exhibited DNA changes years before standard blood tests detected any progression. New groups of abnormal cells emerged and grew over time. While nine participants developed acute myeloid leukemia (AML), their progression patterns varied.

Genetic Insights

The study noted that some MPN cells accumulated mutations gradually. In others, leukemia arose from a different group of abnormal blood cells. Similar patterns appeared in myelofibrosis cases.

Hydroxyurea, commonly used to control blood counts in MPN patients, left small DNA change patterns. No evidence linked this drug to leukemia. A comparable pattern emerged with azacitidine, used to treat certain blood cancers. Genetic changes surfaced years before routine tests detected disease progression.

Implications for Patient Care

Three individuals with “triple-negative” essential thrombocythemia showed no genetic cancer evidence. This finding opens the possibility that some diagnosed patients may not have malignant diseases and could avoid long-term cancer treatment.

An oncologist highlighted that these blood cancers might indicate their future path in DNA long before symptoms appear. This insight is progressing toward precision medicine in blood cancer care.

The study’s small sample size is a limitation. It shows associations rather than proving cause and effect.

Future Directions

The research hints at using periodic DNA re-testing to provide early warnings of disease direction, potentially preceding routine blood work detection.

Future steps include combining early genetic detection with drugs targeting specific mutations, potentially preventing or delaying progression. The immediate potential is for closer monitoring, not new treatments.

A patient with a high-risk mutation might undergo more frequent blood counts and bone marrow exams to catch progression earlier. Stem cell transplant evaluations would remain reserved for those already progressed to myelofibrosis or AML.
