Respiratory therapist Marcy Freeman sees the grim reality of black lung through over 150 funeral programs from her pulmonary clinic in Norton, Virginia, each a testament to a life lost to the disease. These represent hardworking miners who provided for their families, only to fall victim to black lung.

Freeman has treated hundreds of miners with complicated black lung at her clinic, including her husband, a veteran miner of 27 years. New data published in the American Journal of Respiratory and Critical Care Medicine reveals a worsening situation. The National Institute for Occupational Safety and Health (NIOSH) indicates underground miners in central Appalachia face the highest rates of black lung in nearly five decades. Public Health Watch and NPR reviewed a draft of NIOSH’s findings, which show one-third of miners in Kentucky, Virginia, and West Virginia with over 25 years underground testing positive for the disease.

In 2018, the rate was 20.6%; now, even miners with as little as 15 years underground are diagnosed at rates not seen in 40 years. Lead author Scott Laney, a NIOSH epidemiologist, expressed disappointment, noting the severity of the current situation for miners treated like dispensable assets. Between 2020 and 2023, more than 1,700 miners died from black lung, underscoring the urgent need for change.

Celeste Monforton, a workplace safety advocate, emphasized the findings should alarm those who care about coal miners and their families. Despite past regulations meant to curb exposure to coal dust, prevalence rates have risen. NIOSH screened 280 veteran miners in central Appalachia, resulting in a sample estimate of disease rates for the region. This approach, consistent since 1974, reveals a troubling return to conditions like those in 1978.

The study substantiates concerns over silica dust exposure, a dominant contributor to contemporary disease patterns. Radiologist Brandon Crum, who’s been tracking alarming rates of severe black lung, confirms the persistence of these trends. While the National Mining Association recognizes compliance with silica exposure limits, miner advocates remain worried about ongoing risks.

As coal mining shifts to thinner seams, miners face increased silica exposure due to the prevalent quartz in Appalachian rock. Excessive inhalation of coal dust remains the primary cause, but silica’s role is critical. NIOSH confirmed silica presence in respiratory tissues of miners, emphasizing the need for more stringent measures.

Despite compliance rates with dust exposure limits, the risks remain. Public Health Watch found thousands of instances of silica dust overexposure. NPR and FRONTLINE documented similar findings, with retired miners suffering from advanced black lung. Regulatory delays have impeded protective measures despite new regulations established in 2024 aimed at reducing overexposure.

The delay in tougher silica dust regulation enforcement is noted by NIOSH, predicting more disease could result. Dr. Drew Harris of Stone Mountain Health Services, where miners seek testing and benefits claims, highlights these findings as both shocking and anticipated. Miners, often deprived of a voice and broader support, struggle for benefits. The federal black lung trust fund supports thousands, but financial strains remain.

Lung transplants offer little hope for prolonged survival, as demonstrated by miners like Tim Balthis. His advanced stage of black lung imposes immense challenges, making everyday tasks difficult. While some miners might qualify for lung transplants, survival expectations are bleak. Balthis acknowledges the toll on his life but maintains hope, driven by faith and commitment to his loved ones.
